1. Failure to ensure gate was properly secured to prevent accidental swinging.

2. Failure to ensure gate latch was in working condition to prevent it from swinging.

3. Failure to warn traffic along the Causeway of possibility of gate swinging open.

4. Failure to provide proper supervision in ensuring gate was properly secured.

5. Failure to ensure manpower was deployed to check daily on gate.
